Dear Parents,

Below are the items necessary for enrolling your child in school in USD 293. There are also new requirements from the State of Kansas for immunizations.  These new mandates go into effect this fall for the 2011- 2012 school year. 

 

Tdap (tetanus, diphtheria, pertussis); 1 dose is required for students entering 7th- 9th grades regardless of interval since the last Td vaccine.

Varicella (chickenpox); 2 doses are required for kindergarten through 2nd grade and 7th grade.  1 dose is required for students in grades 3-6 and 8-11 unless there is a history of the disease documented by a physician. 2 DOSES OF VARICELLA RE RECOMMENEDED FOR ALL GRADES.

Hepatitis B; 3 doses are required for all students through grade 11.

MMR (measles, mumps, rubella); 2 doses are required for all students.

Poliomyelitis (IPV/OPV); 4 doses required for all students.  3 doses are acceptable if dose 3 was given on or after the 4th birthday.

 

For students less than 5 yrs of age:
Haemophilus influenza type b (Hib); 3 doses
Hepatitis A; 2 doses
Pneumococcal conjugate (PCV7); 4 doses


Preschool Immunizations: If your child is less than 5 years old, he/she will need to receive and have documentation of “up to date” immunizations.  Please bring the records with you to enrollment, or bring them to the elementary school office earlier, and they will be added to your child’s file.

School age Immunizations: If your child is 5 yrs old or older and
you have not yet taken your child for his/her “school age” immunizations, please do so at your earliest convenience. Please bring the records with you to enrollment, or bring them to the elementary school office earlier, and they will be added to your child’s file.

Immunization Records: This needs to have the dates of all immunizations that your child has received.  Immunizations should be “up to date” for the age of your child.

Birth Certificate: If you do not have a birth certificate, please contact the Bureau of Vital Statistics of the state in which your child was born for an application form. (A souvenir hospital certificate is not a legal document for the health record.)

Physical: Students 8 yrs old and younger, enrolling in a Kansas school for the first time, are required to have a physical by a Kansas physician or Kansas health department.  This may be completed no more than 6 months before enrollment. A form is enclosed.

All students in grades 7, 8, and 9, as well as high school students participating in extra curricular activities, or advanced P.E. are required to have a KSHSAA physical completed prior to enrollment.  This form is available at the high school office.

Health History: A form is enclosed and needs to be completed prior to enrollment if you have not filled one out previously.
